Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, which means the same neural receptors which cause an individual to feel high from heroin or prescription pain killers are stimulated by these two drugs so that an opiate dependent client will instead no longer become involved in drug seeking behavior and won't experience cravings and withdrawal. There are risks of side effects, drug interactions, and even addiction and overdose with both buprenorphine and methadone, which is why they are heavily regulated and can only be dispensed in a clinical setting.
Buprenorphine and methadone clients are on these medications daily so they don't experience withdrawal symptoms, etc. but hopefully at a dosage which isn't also making them feel euphoria. Euphoria is possible at at higher dosages, more significantly with methadone than buprenorphine. At centers that treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are generally limited beyond managing the medication and the primary treatment objective is to handle the physical symptoms of their dependence. Some programs do offer a small amount of counseling and therapy as secondary treatment, and this is recommended and encouraged.
