There are programs available in or near Wailuku which service both methadone and buprenorphine clients only, drugs which are dispensed in what is known as opioid maintenance therapy. Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, meaning the same neural receptors which cause an individual to feel high from morphine or heroin are stimulated by these two drugs so therefore an opioid addicted client will ideally not engage in addict behavior and won't have withdrawal symptoms. There are risks of side effects, drug interactions, and even overdose with both methadone and buprenorphine, which is why they are highly regulated and can only be dispensed in a clinical setting.
Methadone and buprenorphine clients take these medications consistently so they don't experience cravings, withdrawal, etc. but hopefully at a dose which isn't also making them feel euphoria. Euphoria occurs at a high dosage, more significantly with methadone than buprenorphine. At centers that treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond managing the medication and the primary treatment objective is to address the physical symptoms of their dependence. Some programs do offer a small amount of counseling and group therapy as supplementary treatment, and it is encouraged.
