There are programs offered in or near Rockbridge which service both methadone and buprenorphine clients only, drugs that are used in what is called opioid maintenance therapy. Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, which means the same neural receptors that cause an individual to feel high from morphine or heroin are activated by these two drugs so therefore an opiate addicted client will instead no longer become involved in drug seeking behavior and won't experience cravings and withdrawal. There are dangers with side effects, drug interactions, and even addiction and overdose with both methadone and buprenorphine, which is why they are highly regulated and can only be administered in a clinical setting.

Methadone and buprenorphine clients take these medications daily so they don't experience withdrawal symptoms, etc. but ideally at a dosage which isn't at the same time causing them to feel euphoria. Euphoria can be experienced at a high dosage, more significantly with methadone than buprenorphine. At programs that treat methadone and buprenorphine clients only, treatment therapies are limited beyond managing the medication and the main treatment objective is to treat the physical symptoms of their dependence. Some centers do offer a small amount of counseling and group therapy as auxiliary treatment, and it is encouraged.
