Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, meaning the same nerve cell receptors which make a person feel high from morphine or heroin are stimulated by these two drugs so that an opioid addicted client will ideally not engage in addict behavior and won't experience withdrawal symptoms. There are dangers with side effects, drug interactions, and even overdose with both buprenorphine and methadone, which is why they are highly regulated and can only be dispensed in a clinical setting.
Methadone and buprenorphine clients take these medications regularly so they don't experience cravings, withdrawal, etc. but hopefully at a dose which isn't at the same time making them feel euphoria. Euphoria occurs at at higher dosages, more so with methadone than buprenorphine. At clinics which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond this and the main treatment objective is to handle the physical symptoms of their dependence. Some centers do provide a small amount of counseling and group therapy as supplementary treatment, and this is recommended and encouraged.
