Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, meaning the same nerve cell receptors that make a person feel high from morphine or heroin are stimulated by these two drugs so therefore an opioid dependent client will ideally not become involved in drug seeking behavior and won't experience cravings and withdrawal. There are dangers with side effects, drug interactions, and even overdose with both methadone and buprenorphine, which is why they are highly controlled and can only be dispensed in a clinical setting.
Methadone and buprenorphine clients take these medications daily so they don't experience cravings, withdrawal, etc. but ideally at a dose which isn't at the same time making them feel high. Euphoria can be experienced at a high dosage, more significantly with methadone than buprenorphine. At programs that treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond managing the medication and the primary treatment goal is to treat the physical symptoms of their dependence. Some programs do deliver a small amount of counseling and therapy as auxiliary services, and it is encouraged.
