Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, which means the same neural receptors that make a person feel high from morphine or heroin are stimulated by these two drugs so that an opiate addicted client will ideally no longer engage in drug seeking behavior and won't have withdrawal symptoms. There are risks of side effects, drug interactions, and even overdose with both methadone and buprenorphine, which is why they are highly regulated and can only be dispensed in a clinical setting.

Buprenorphine and methadone clients are on these medications regularly so they don't experience withdrawal symptoms, etc. but hopefully at a dose which isn't also making them feel high. Euphoria is possible at at higher dosages, more so with methadone than buprenorphine. At programs that treat methadone and buprenorphine clients only, treatment therapies are limited beyond this and the primary treatment goal is to treat the withdrawal symptoms and put an end to the illicit drug use. Some clinics do provide a small amount of counseling and therapy as auxiliary services, and this is recommended and encouraged.
