Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, which means the same nerve cell receptors which cause an individual to feel high from morphine or heroin are stimulated by these two drugs so that an opiate dependent client will ideally not become involved in addict behavior and won't have cravings and withdrawal. There are dangers with drug interactions, side effects, and even addiction and overdose with both buprenorphine and methadone, which is why they are highly controlled and can only be administered in a clinical setting.

Buprenorphine and methadone clients are on these medications daily so they don't experience withdrawal symptoms, etc. but ideally at a dosage which isn't also causing them to feel euphoria. Euphoria occurs at a high dosage, more significantly with methadone than buprenorphine. At programs which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are generally limited beyond this and the primary treatment objective is to manage the physical symptoms of their dependence. Some clinics do deliver a small amount of counseling and therapy as auxiliary services, and it is encouraged.
