Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same neural receptors which cause an individual to feel high from morphine or heroin are activated by these two drugs so that an opiate dependent client will instead not engage in addict behavior and won't have withdrawal symptoms. There are risks of side effects, drug interactions, and even addiction and overdose with both methadone and buprenorphine, which is why they are highly regulated and can only be dispensed in a clinical setting.
Methadone and buprenorphine clients are on these medications regularly so they don't experience cravings, withdrawal, etc. but ideally at a dose which isn't at the same time making them feel euphoria. Euphoria occurs at a high dosage, more so with methadone than buprenorphine. At programs which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are generally limited beyond managing the medication and the main treatment objective is to treat the withdrawal symptoms and put an end to the illicit drug use. Some clinics do deliver a limited amount of counseling and group therapy as secondary services, and it is encouraged.
