There are substance abuse programs offered in or near Tar Heel that cater to both methadone and buprenorphine clients only, medications that are dispensed in what is known as opioid maintenance therapy. Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same nerve cell receptors that make a person feel high from heroin or prescription pain killers are activated by these two drugs so therefore an opiate dependent client will ideally no longer become involved in drug seeking behavior and won't experience withdrawal symptoms. There are risks of drug interactions, side effects, and even overdose with both methadone and buprenorphine, which is why they are highly regulated and can only be dispensed in a clinical setting.

Buprenorphine and methadone clients are on these medications daily so they don't experience cravings, withdrawal, etc. but ideally at a dose which isn't at the same time making them feel high. Euphoria can be experienced at a high dosage, more significantly with methadone than buprenorphine. At centers which treat methadone and buprenorphine clients only, treatment therapies are limited beyond managing the medication and the main treatment objective is to address the physical symptoms of their dependence. Some centers do deliver a small amount of counseling and therapy as auxiliary services, and this is encouraged.
