There are programs available in or near Scotland that cater to both methadone and buprenorphine clients only, controlled medications which are used in what is called opioid maintenance therapy. Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, meaning the same neural receptors that make a person feel high from heroin or prescription pain killers are stimulated by these two drugs so that an opiate dependent client will ideally no longer become involved in drug seeking behavior and won't have withdrawal symptoms. There are dangers with drug interactions, side effects, and even addiction and overdose with both buprenorphine and methadone, which is why they are highly controlled and can only be administered in a clinical setting.

Methadone and buprenorphine clients are on these medications regularly so they don't experience cravings, withdrawal, etc. but hopefully at a dosage which isn't at the same time causing them to feel euphoria. Euphoria is possible at a high dosage, more so with methadone than buprenorphine. At programs that treat methadone and buprenorphine clients only, treatment therapies are limited beyond managing the medication and the main treatment goal is to manage the physical symptoms of their dependence. Some clinics do provide a limited amount of counseling and therapy as secondary treatment, and this is recommended and encouraged.
