Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, which means the same neural receptors that make a person feel high from morphine or heroin are stimulated by these two drugs so that an opioid dependent client will instead not become involved in drug seeking behavior and won't have withdrawal symptoms. There are dangers with side effects, drug interactions, and even addiction and overdose with both methadone and buprenorphine, which is why they are highly controlled and can only be administered in a clinical setting.
Buprenorphine and methadone clients are on these medications regularly so they don't experience cravings, withdrawal, etc. but hopefully at a dosage which isn't at the same time making them feel high. Euphoria occurs at at higher dosages, more significantly with methadone than buprenorphine. At clinics which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are generally limited beyond managing the medication and the main treatment goal is to manage the physical symptoms of their dependence. Some programs do provide a limited amount of counseling and therapy as auxiliary services, and this is recommended and encouraged.
